Measure: Potentially Preventable ED Visits for people with Behavioral Health diagnosis (PPV-BH)
Focus: Reduce PPV-BH for all Herkimer County residents with 2 or more ED visits by 30% by June 30, 2018. As of Feb 2017, 36 patients make up 129 visits. The following approaches were identified:
- Establish 1 patient making up 16 visits with a PCP closer to their residence
- Enroll 22 patients making up 58 visits with Medicaid Health Home
- Enroll 13 children making up 36 visits with Children’s Medicaid Heath Home.
