This is a New York State Initiative to Improve Health Care Delivery
We are the Leatherstocking Collaborative Health Partners (LCHP), formed by providers from Delaware, Herkimer, Madison, Otsego and Schoharie counties.
On April 14th, 2014 Governor Andrew M. Cuomo announced that CMS (Center of Medicaid and Medicare Services) approved and finalized New York’s waiver request. Due to this approval, the State can now reinvest the $8 billion generated by Medicaid Redesign Team (MRT) reforms. The Delivery System Reform Incentive Payments (DSRIP) Program will use these dollars for a performance based sustainable transformation of health care throughout the state. Under the DSRIP Program, various providers are coming together to collaborate and provide better care to the patients. LCHP’s is eligible to receive $71,839,378, provided all requirements are achieved in the designated time.
LCHP’s Performing Provider System (PPS) currently has about 98 partners enthusiastic about transitioning health care towards patient centered care, collaboration, better access and reduced costs of health care.
Key components of the DSRIP program include:
- Community needs assessment
- Partner selection in the development of PPS
- Project selection
- Project evaluation
- Attribution of Medicaid enrollees
- Performance metrics
- DSRIP payment allocation among members
