Measure: Potentially Preventable ED Visits for people with Behavioral Health diagnosis (PPV-BH)
Focus: Decrease PPV-BH for residents of Madison county south of route 20 by 30% by June 30, 2018. As of Feb 2017, 6 patients have 20 visits in the identified population.
Measure: Access to Care
Focus: Educate primary care providers in Madison County about the other available services in the area they can refer their patients as needed, which in turn will improve patient access.
