Measure: Potentially Preventable ED Visits for people with Behavioral Health diagnosis (PPV-BH)
Focus: Reduce PPV-BH for residents in the City of Oneonta with 2 or more visits by 52 visits(50%) by June 30th, 2018. As of Feb 2016 claims data, 30 patients had 104 visits with the above criteria. We will reduce these visits by connecting eligible patients with a Health Home, Light Navigation or SPOA for Otsego County.
Measure: Cross Metric Patients
Focus: Patients within Otsego County who are in the denominator of multiple Performance Measures. Workgroup will focus on the 7 patients who fail the most measures for lower is better scores. As of May 2016 claims data, 7 patients had 84 visits with the above criteria. We will reduce these visits by 30 % by June 30th, 2018.
Social Determinants of Health:
Focus: Provide needed services including transportation, food, and educational programs in an impoverish community identified in Otsego County by establishing a community center.
