Focus: Reduce PPV (BH) for residents of Schoharie County with 3 or more visits by 35% (68 visits) by June 30th, 2018. As of Feb 2016 claims data, 41 patients had 192 visits with above criteria.
Measure: Diabetes screening for people with anti-psychotic medications
Focus: Increase diabetes screening for patients with antipsychotic drugs in Schoharie County by 90% (36 patients) by June 30th, 2018. As of May 2016 claims data, 40 patients with antipsychotic medications had no diabetes screenings.
